Safety Observation Report

Safe Condition: Creating a work space which greatly reduced the risk of a potential incident.

Near Miss: A potential hazard or incident in which no property was damaged and no personal injury was sustained. However, if given a slight shift in time or position, damage or injury could have easily occurred.

Unsafe Act: The violation of a safety procedure which could cause an incident.

Unsafe Condition: A hazardous condition or circumstance that could directly cause an incident.

Diamond Near Miss: An event that occurs where the results could have resulted in severe injury or fatality.
